Hepatic vein -flow reversal




Hepatic vein flow reversal happens in many conditions like Severe Right heart decompensation,cirrhosis etc.
On spectral Doppler,we can clearly see the flow reversal happening and on color Doppler , we can see the alternating flow towards the probe(Red) and flow away from the probe(Blue).

Assessment of pulmonary trunk can be done by two windows.a)parasternal short axis view.b)subcostal pulmonary window.you can clearly see the pulmonary trunk dilatation with increased Trans pulmonary velocities

Meta-analysis of high- versus low-chloride content in perioperative and critical care fluid resuscitation M. L. Krajewski etal.

"Fluid resuscitation with  large quantities of high chloride containing fluids are shown to increase the incidence of AKI , hyperchloremic normal anion gap metabolic acidosis and mechanical ventilation times,thereby poor outcomes"

fluids for  resuscitation in critically ill patients  is a controversial topic.Multiple RCT'S have shown harmful effects of hydroxy ethylstarchs.Having said that,fluids are the cornerstone for capillary recruitment in microcirculatory dysfunction syndrome to maintain adequate capillary driving forces .current evidence support crystalloids over colloids for fluid resuscitation in critically ill patients.There is insufficient data or RCT's comparing Normal Saline vs RL (Crystalloids) and also High chloride vs Low chloride containing fluids for resuscitation.Most of these studies are retrospective, prospective or observational  with variable quality ,selection bias and some  RCT's with small sample size with conflicting results making it more complicated .

 "This study shows that there is no difference in the mortality at 30 days and as well at 90 days  between both the early  parenteral & enteral nutrition arm & no difference in mechanical ventilation days,incidence of infections .On the contrary,enteral group had more hypoglycemic events,vomiting & gastric intolerance needing more prokinetics.On the other hand, EPNIC Trial showed increased mechanical ventilation days,increased incidence of infections,low hypoglycemic rates in early parenteral group with no difference in 90 day mortality in either of the groups"

" Author attributes  the reason for less complications with parenteral nutrition to better and advanced vascular techniques ,better VAP and CRBSI  prevention protocols,Better and efficient parenteral nutrition deliver methods,monitoring on parenteral nutrition, which was not considered in the previous trials" .
